This position is based in Swan Hill.

The Initial Assessment and Planning (IAP) Worker will act as the first response and access point for Mallee Family Care in Swan Hill and broader Southern Mallee Region. The position will ensure  that Mallee Family Care provide a “No Wrong Door” coordinated approach and ensure that all individuals and families have their immediate needs responded to, and where required a seamless transition to longer term support or other solutions that address need and/or prevent/resolve crisis. This position will oversee the initial assessment and enquires and provide advice, information, referral, short term case management and other practical assistance.  The position will be responsibility to develop and enhance integrated practices internally and externally, and work closely in partnership with reception services to deliver high quality customer services.

The successful applicant will be required to hold an accredited qualification relevant to the position at Degree level or above.
